FORMS

All forms are mandatory and must be completed prior to tryouts. All athletes must complete the forms found in the parent portal at RankOneSport.

These forms include Physical, Medical History, UIL Forms, and Contact Forms. Complete all AISD Participation Forms through the parent portal at RankOneSport. Access RankOne Portal →
A current AISD Physical Form is required for all student-athletes. This must be completed by a healthcare provider and submitted before participation. Download Physical Form (PDF) →
All players must complete and submit the Private Transportation Permit before they can travel to practices or tournaments independently. Download Transportation Permit (PDF) →
An Emergency Student Information Card must be on file with the coaching staff. This ensures we have critical contact and medical information for all athletes. Download Emergency Card (PDF) →
